New Patient Registration
Generally we will only accept patients living within our defined practice area; details are available in reception. New patients registering with the practice will be asked to complete a “Health Questionnaire” and will be invited to attend a health check appointment if they are on regular medication. Patients are registered with the practice, rather than an individual doctor. However, patients can express a preference of practitioner. We will try to comply with your request, but it might not always be possible, in which case we will explain the reasons.
In addition if you are unable to attend the surgery for a health check because of your medical condition a home visit may be arranged.
We ask you for personal information to enable us to give you the very best care and treatment. This information is recorded on the computer and we are registered under the Data Protection Act. All members of staff are bound by strict rules of confidentiality.

How to Register
If you wish to register with the practice you need to fill out the below registration forms and return them to the surgery with proof of your address. At the bottom of the page is an example of how the registration form should be completed. We are not able to register you with the practice until the form is filled out correctly.
Some examples of proof of address accepted by the practice are:
- Utility Bills
- Council Tax
- Tenancy Agreement
In order to register you we ask that you fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this form too, which you may fill out and return to the surgery.
Please fill out one of each the below forms for every member of the household you wish to register with the practice.
All forms need to be completed, signed and returned to Pontprennau Medical Centre in order for you to be registered with the practice.
